  1. Please fill out the Lead Teacher Agreement form and submit it to us prior to your visit.
  2. Chaperone Agreement: Please print and bring the completed agreement with you on the day of the field trip.
  3. Roll Call Sheets: Please print and bring the completed roll call sheets with you on the day of the field trip.
  4. Transportation/Bus Information: Please print this information and provide it to your bus driver.
